02 / Certificate holders

Use this exact order

Issue separate certificates when requested. If a combined certificate is accepted, preserve this order.
01

PROSPER (Inside Marketing Inc.)

Must be listed first on a combined certificate.

02

Seamless Home LLC

1441 W. Ute Blvd., Ste. 270, Park City, UT 84098

03

GoodLeap

8781 Sierra College Blvd., Roseville, CA 95661

03 / Endorsements

The COI is not enough by itself

Policy endorsements must substantiate the certificate wording.

Ongoing and completed operations

PROSPER (Inside Marketing Inc.) must be listed first, followed by Seamless Home LLC and GoodLeap where applicable, as additional insureds for ongoing and completed operations.

Submit ISO forms CG 20 10 and CG 20 37, or equivalent endorsements that substantively provide both coverages.

Workers' compensation exceptions require written review

Do not self-determine an exception based on a 1099 label, sole-proprietor status, another company's policy, or a state name. Coverage under another entity is not accepted unless PROSPER Compliance and the issuing insurer or agent approve the exact arrangement in writing before work begins.
